Requirements:
The applications will consist of:
• High School Transcript (minimum of 75% or 2.5 GPA).
• List of community service projects.
• Three letters of recommendation. Two letters should be from teachers and one from a contact within the community. All recommendations must be filled out by non family members.
• Two essay questions. Questions are below.

Essay Scholarship/Award Questions
Please answer two of the following essay questions. Question 1 is mandatory. You then have a choice of the remaining questions for your second essay. Answers must be typed and double spaced.
1. What do you feel most passionate about? Give an example of how this passion involves living your life for others.
2. Can you think of a magazine ad, TV commercial or current social issue that exemplifies society’s definition of success? What message is it trying to tell you? How do you define success in your life?
3. Describe a specific example where your individuality was celebrated more than the concept of the team/group of which you are/were a part. How was this message conveyed? Should/How should this message be changed by your coaches, teachers, mentors, or leaders?
4. What are three major challenges facing your community? What action can you take to help resolve these community challenges?
5. Define empathy. Give an example from your life where having empathy changed your mind or outlook about a person or situation.
